Admissions Process and Information
If you have found your way to this page, something spoke to you or there is a part of you that would like to find out more about enrolling yourself or a loved one into EVNSQ. This is a big decision, and we are here to guide you through the entire process.
Call our Admissions team at (801) 851-5260 anytime and speak with Cameron Cann or Mark Oerther, who is also a partner at EVNSQ. Cam or Mark will walk you through your questions and get a better understanding of your current situation.
Should you decide EVNSQ is a good fit for you, we have an application for you to fill out, which will allow you or your loved one to get clinically and medically approved for the program.
We do not claim to have all of the answers to everything. However, we do have some. We promise to make you feel safe, well taken care of, and above all else a part of our community. Our goal is to make this as smooth of a transition as possible for you and your family.
We are located approximately 20 minutes from the SLC Airport. If you are approved for the program, we will discuss an arrival date and logistics.
